Summary: Druuna is keeping Schastar hidden in her apartment, and keeping his disease under control by trading sex for serum with Dr. Ottonegger. A little man takes Druuna to a lower level to see a non-contagious hermaphrodite mutant, who tells her that Schastar has some knowledge that will help them all. This seems to ring a bell with Schastar, and he takes Druuna on an elevator ride to an upper level, the "city above." As they arrive the sickness gets too strong and Druuna has to shoot Schastar through the heart. In the upper level, Druuna meets the head of the original commander of the city ship. Delta, the computer, keeps the head alive by extracting something from the bodies of healthy humans stored in tubes. The head tells Druuna that Delta has gone psychotic and is planning to self-destruct, and the commander is trying to act independently and save the situation.
